Part-Time Telehealth Opportunity for RN Licensed in Michigan

The RN Care Manager is a licensed professional providing virtual oversight of patients participating in our client’s home-based Remote Patient Monitoring (RPM) program.

The RN Care Manager will provide patient education, support, and interventions via telephone and/or video to patients who are participating in this RPM program. This healthcare professional will work with the patient to activate and use assigned at-home devices, comprehensively understand their medical, behavioral, and social needs, and co-create a care plan in tandem with the patient to help them reach their healthcare goals. The RN Care Manager also collaborates with the patient's provider, caregiver, or other care team members for the provision of services and seamless care based on the unique needs of the patient.

Responsibilities:

  • Establish vital sign parameters for assigned patients per our client’s clinical model, policies and protocols.
  • Ensure documentation supports administrative functions such as quality management and billing.
  • Follow escalation protocols to appropriately notify other clinical care team members of changes in patient condition or status.
  • Develop and maintain rapport with patients, caregivers, and care team members via phone and/or video communication.
  • Assess and evaluate patient data and needs, providing appropriate support and education.
  • Identify the need for and recommend appropriate programs, services, or community resources to promote optimum patient care.
  • Maintain clinical skills within nursing practice norms.
  • Demonstrate a professional attitude, sound judgment, and continuity in interactions with physicians, other healthcare providers, patients and caregivers.
  • Be an integral part of a team of passionate healthcare experts spread heading the development of innovative, low risk/high benefit programs to keep patients healthy, happy, and out of the hospital!

Requirements:

  • Clinical Licensure:
    • Current and unobstructed RN Licensure in Michigan

Knowledge and skills:

  • Skilled in nursing processes.
  • Excellent clinical acumen.
  • Empathy, patience, and passion for delivering a phenomenal patient experience.
  • Exceptional written, verbal and interpersonal communication skills.
  • EHR/CMS experience
  • Highly personable, social telephone manner.
  • Work well as a team member

Working Hours:

  • Commitment to a minimum of 40 hours per month of pre-scheduled shifts during Monday to Friday from 8:00 a.m. to 8:00 p.m local time.
  • Appointments will be conducted as scheduled synchronous video, telephone visits, or a-synchronous.
